Requests from Colleges and Faculties must be submitted by the authorised College/Faculty Officer e.g., Head of Exams/Certification. Please ensure that any requests raised on this form have already been discussed with the relevant Postgraduate Dean (or nominated deputy), and that the request outlines what specific additional advice is being sought. Please note that the relevant Postgraduate Dean/Lead Dean or Liaison Dean will be copied into the response from the Gold Guide Reference Group. Requests from Postgraduate Doctors in Training will not be accepted, however they should be discussed with the educational supervisor or training programme director and submitted by the relevant education faculty/programme management team.
